Travel vaccination enquiry forms should be completed 6-8 weeks before intended date of travel.
Forms cannot be processed before 6-8 weeks of travel date due to potential changes in requirements in other countries.
Any forms received without 6-8 weeks notice may not be processed as we will not have enough time to get an appointment booked or the level of protection required from a vaccination may need specific time periods to take full effect.
Vaccination schedules can be complicated so the information requested on this form assists us to work out your specific requirements.
